Every woman, from teenager to first time expectant mother to the 50-year- old embarking upon the menopause has many questions. All of them are important, all of them are a vital component of your total health care needs. We encourage you to prepare for your exam as you would an interview: list your questions prior to your visit with our doctors and nurses. TELEPHONE CALLS: When calling the office, please indicate the nature of your problem or question to our receptionist who will expedite your call to our nurses. Three items of information from you are vital: - YOUR NAME including any recent name, address or insurance coverage changes, - YOUR PRESCRIPTION NUMBER AND PHARMACY TELEPHONE NUMBER, INCLUDING AREA CODES for prescription refills, - YOUR TELEPHONE NUMBER INCLUDING AREA CODE. ALL MEDICATION REFILLS MUST BE DONE DURING REGULAR OFFICE HOURS. Prescription refills and medications cannot be re-filled or phoned to the pharmacy after 4:00 p.m. or through an after-hours phone call. In case of an emergency or labor after office hours, contact Dr. Evans' 24-hour answering service at 214-739-1300. In rare cases, in case the telephone service fails, you may need to contact Presbyterian Hospital labor and delivery directly at 214-345-2653, if you are pregnant. if you're not pregnant, the emergency room number is 214-345-7886. HOSPITALS: Although they have privileges at several local hospitals, Dr. Evans works primarily at Presbyterian Hospital of Dallas. He is also an Associate Clinical Professor of OB-GYN at the University of Texas, Southwestern Medical School, Dallas. PATIENT EDUCATION: Our office is a vast resource of information on virtually every aspect of women's health care. We also retain a video library to help explain certain procedures and complications. But the best information comes directly from Dr. Evans and the caring staff. They are all trained and available to help you find the answers to your health care questions or needs. FEES: All medical services are the financial obligation of the patient or responsible party, such as an insurance company or managed care health plan. Fees for office calls, procedures, lab work and exams are to be paid at the time services are rendered. For pregnancies and some surgeries, we accept assignment from your insurance company. Since most companies do not pay the entire cost of such care, we do require a deposit which must be paid in full before the beginning of the seventh month of pregnancy or before surgery. If the payment from the insurance company exceeds the fee, we will mail you a refund within 30 to 45 business days. With managed care plans, excess payments will be held for refund to the insurance company. Returned checks must be paid in cash. A $50.00 fee is charged for all returned checks. We accept Visa, Mastercard, American Express,Discover and most major credit card payments for your convenience. USUAL AND CUSTOMARY FEES: Insurance companies often take a lump sum average of physician charges in specified geographic areas. Often the average fee figure is lower than that of the actual charge, because the companies are mixing doctor's fees from rural, suburban and metropolitan areas. Dr. Evans' fees are competitive with the fees of other physicians in the North Dallas area at Presbyterian Hospital of Dallas. INSURANCE AND MANAGED CARE: Due to the many changes in the way health care is financed and delivered, we are required to obtain certain information from our patients even before they see the doctor. All patients must have their insurance coverage pre-certified by our staff prior to their office visit. We must have your most recent insurance information on file in our office to qualify for benefit assignment. Patients are financially responsible for deductibles and the balance not covered by their policies. Co-pays must be paid at the time of every visit. By updating your insurance information, we can help you obtain the coverage you deserve from your specific policy. Patients who belong to PPO's or HMO's must provide our office with a referral number from a primary care physician before they can be examined by Dr. Evans. Some plans require a referral from the primary care physician in the form of an office visit prior to being examined by a gynecologist or obstetrician. In the complicated maze of health care options and plans, it is in the patient's best interest to understand her insurance benefits and coverage. Our business staff will be happy to answer your questions, as will the staff of your insurance carrier or the benefits department of your employer. We allow 45 days for assignment claims to be paid by the insurance company. After 45 days, the total unpaid balance is due from the patient or responsible party. Should your employer or Insurer become unable to pay your claims, you the patient are ultimately responsible for payment of your medical expenses. You may also contact the Texas Department of Insurance for further assistance or information: REQUESTS FOR ITEMIZED STATEMENTS AND MEDICAL RECORDS: When you check out of our office, the receptionist will provide you with a charge and payment receipt. Please retain this document to file your insurance and for income tax purposes. Should you need additional copies of your financial records or chart, there will be a small additional charge. In the interest of reducing office paperwork and time, we are trying to minimize your health care costs as much as possible as well as preserve the beauty of nature's trees. OUR WISH: To provide you with the best, most dedicated and loving health care you can find. Many working mothers and busy women today have little time to care or themselves as carefully as they should. Dr. Evans is your very own physician who cares about you! CONFIDENTIALITY: All patient's medical records are strictly private. No information regarding your condition and treatment will be given to anyone without your written consent. We also do not share medical records with any database at this time to protect and respect your privacy. YOUR PERSONAL PHYSICIANS: Dr. Evans has been practicing obstetrics and gynecology for over 23 years. Dr. Evans shares weekend call with four other board certified OB-GYNs at Presbyterian Hospital of Dallas: Dr. Ali Toofanian, Dr. David Fein, Dr. Felicia Wagman and Dr. Nathan Thomas. If you are expectating a baby, Dr. Evans will be your primary obstetrician throughout your pregnancy and most likely your delivery. Their qualified staff are available when the doctor is in delivery to support you and answer questions. We believe the best health care and delivery experience is achieved by knowing, understanding and respecting each patient as a unique individual. That goal is shared by all our call-sharing doctors.
